Shipment Request

If you are purchasing art for shipment only:

Please provide:

• the art ID# and title
• recipient name and address where you would like it shipped to
• your email contact info to confirm information and send invoice to (a copy of this information will be sent to you before shipment is made)
• By submitting this response form, I agree that this is a non-refundable purchase.
• Any art returned to me by the delivery service or courier for any reason (including address errors made by you) will be returned to the gallery for resale to future patrons.
• You will be notified of the shipment return and if you wish to have the art sent to a new address, additional shipping charges will apply before art will be sent out as a new shipment.
